Use this request form if you have a product that requires repairs or an assessment for warranty coverage. Please provide as much information as possible in order to expedite the process. A bkm service team member will be in touch within 24 business hours. 
  • Provide the name of your Account Manager or most recent contact at bkm (if known).
  • CHAIRS: To find order #, flip chair over and look for white tag. On tag look for either order # or job #.
  • CHAIRS: To find model #, flip chair over and look for white tag. On tag look for either model # or item #.
  • Please be as detailed as possible. The more information you provide, the faster we can assess the issue at hand and take action.
  • The warranty / repair process can be significantly expedited when you submit photos of the product that needs attention. Please submit as much information as possible.
